Syringomyelia: General
Chiari malformation, cervical disc prolapse and syringomyelia - always think twice.

When significant spinal canal compression from a herniated disc is associated with syringomyelia, even in the presence of a Chiari I malformation, our approach has been to remove the disc first. Now, the first case report has been published showing resolution of the syrinx following spinal decompression without treatment of the Chiari malformation.

Trauma or infection can cause scarring of the arachnoid membrane. The arachnoid scar can tug (cause traction) on spinal cord and lead to development of a syrinx.

When should a child or adolescent with scoliosis have an MRI to look for syringomyelia? Qui et al. measured the scoliosis curves in 87 children and adolescents with syringomyelia and scoliosis. Over 40% were noted to have atypical curve patterns and 65% with typical curve patterns had atypical features.
The authors conclusion: "These results show that radiographic presentations including atypical curve patterns, atypical features in typical curve patterns, and a normal to hyperkyphotic thoracic spine may suggest the need for a preoperative magnetic resonance imaging.

Large retrospective study from Qilu Hospital of Shandong University in China measuring the clinical outcome and extent of syrinx collapse following surgery. While the response in syrinx size was variable, clinical improvement was noted in 94% of 218 patients available for follow-up.
